New patient forms
Welcome! We’re glad you’re here. For a faster check-in experience, you can fill out your new patient forms before your appointment.
When your forms are filled out, you can choose to:
- Email them to us at info@eyeinstitute.com.
Tip: Be sure to save your completed forms before you save and attach them to the email. - Print them and bring them with you to your appointment.
On the day of your appointment, you’ll just need to bring a list of your current medications, your insurance card, and a driver’s license or another photo ID.

Medical release forms
You can download the forms to have your records sent to the Eye Institute or to request the forms that we have on file be sent to you or another office.
